《香港和澳門特別行政區醫師內地醫師資格認定申請審核表》中華人民共和國衛生部監製本表供香港和澳門特別行政區醫師申請內地醫師資格認定使用。

• 我們收到文件後，將聯絡你安排付款及於香港作一次宣誓／聲明，由中國委託公證人及香港律師監誓

7. 與擬申請醫師資格類別相應的港／澳專科醫師執照或者專科醫師資格證明
   Practising Certificate for Specialists

   HKAM Constituent Colleges (HK College of Anaesthesiologists): "Admission to Fellowship" [Sample]
8. 港／澳醫療機構的在職證明或者執業登記證明

   Certificate of Employment or Registration corresponding to the type of Yishi Zige under consideration in the application

9. 執業期內無不良行為記錄的證明

   Certificate of No Professional Misconduct

10. 港澳居民來往內地通行證（又稱回鄉證）
    Mainland Travel Permit for Hong Kong and Macau Residents [Sample]
11. 居住地址證明（在過去三個月內，與申請中所述的居住地址相同）
    Proof of residential address (within the past three months, of the same residential address as that stated in the application)

Please contact MCHK with reference to the fee schedule for the following documents:

• Certificate verifying registration in the General Register, or inclusion of name in the Specialist Register
• Certified copy of an entry in the General or Specialist Register
• Certificate of good standing

有關「無刑事犯罪記錄的證明（原件）」之要求：按照現行慣例，中華人民共和國衛生行政管理部門要求閣下提供無犯罪記錄證明，唯因香港警務處不基於本申請目的簽發此類證書，香港申請者可以出具經中國委託公證人香港律師公證的聲明，作為「無刑事犯罪紀錄的證明」，heebay 會代為處理，閣下無須理會。In current practice, the PRC Administrative Department of Health requires a Certificate of No Criminal Conviction from the Client. By making use of this Service, the Client understands that the Hong Kong Police Force does not issue such Certificates for the purpose of this Application, and that heebay will arrange for the Client to make a corresponding declaration to that effect.
